For general notarizations, bring all paperwork and form you need signed completely filled out but NOT SIGNED. 

The individual(s) signing the document must appear in person and present a CURRENT valid photo identification.  (Examples include a CURRENT state issued Driver License or identification card, U.S. Passports or military identification cards.) This must be presented by ALL signers prior to any notarization being performed.
